Be Well - Weight Management and Complex Obesity Service (WMCOS) : An integrated pathway that enables coordinated transitions from lifestyle and weight management services to the Complex Obesity Service. Obesity & weight service Be Well – Cancer Awareness Initiatives: Caribbean and African Community Health Support Forum promoting cancer awareness and prevention within Black and Minority Ethnic (BME) communities in Suffolk, addressing the reported disproportionality in certain cancers. Age Well – Suffolk Dementia Strategy: developed involving people with dementia, their carers and families, and key stakeholders including the Voluntary, Community, Faith and Social Enterprise (VCFSE) sector Dementia strategy Age Well – Strength and Balance Programme: to prevent falls by improving the gait, balance, co-ordination, muscular strength, flexibility and confidence of those who have had a recent fall or been identified as being at high risk of having a fall. Strength and Balance programme Outcome: Die Well People at end of life, to have flexibility, choice and control over their care, and support for those closest to them. We want those bereaved to feel supported and to access care when they need it in their own right. Die Well – Compassionate Communities: offers safe spaces where people can openly explore concerns around death, dying, and loss.
