Can Do Health & Care - Thinking Differently Together About Personalised Care

Suffolk and North East Essex Integrated Care System (ICS) 1. Introduction Online event Wednesday 13 October 2021 ‘Thinking Differently Together’ about… Personalised Care We all want and need to be treated as individuals. A consistent issue for many of us when accessing support from health and care services is the need for an approach that recognises our own individual needs and circumstances and also the challenges we face on a daily basis. Our event explored why as a health and care system we need to place personalised care at the heart of everything that we do. Tom Brown, Chief Executive of the Green Light Trust Our independent Chair, Tom Brown, is Chief Executive of the Green Light Trust. Tom explained that the Green Light Trust is an environmental education charity, supporting people from marginalised or disadvantaged backgrounds, using the natural environment as a means to engage and support them. They work with around about 2,000 adults, children and young people each year. As a third sector organisation, what they do very well is to stay in people’s lives long enough to gain their trust and make them feel comfortable with them, and this allows them to help people address some of the underlying issues in their lives. A good example of this is a young lady of 18 who had been disengaged with school since the age of 12. She had support for her mental health, confidence and self-esteem, but what really unlocked her potential was gaining a qualification, which opens up the opportunity for her to live independently, to engage, socialise and give her whole life purpose. Our event focused on person- centred approaches like this, that organisations can provide to make the difference to so many people. 1 | Suffolk and North East Essex Integrated Care System
