Ipswich and East Suffolk Alliance

Our Alliance has a Committee focused on our strategy and performance and an Executive Delivery Group, driving forward development and implementation of our Delivery Plan. Locality-based Core Leadership Teams of Primary Care Network Clinical Directors, Social Care, Community Care, Mental Health leads and Social Prescribing partners are evolving to build on individual joint working initiatives to create holistic plans for our communities; currently informed by local needs assessments but soon to be boosted by population health management tools. We are continuing our journey of aligning our resources and sharing investment decision making, supported by Alliance Financial Strategy and Integrated Quality Groups.

1. Implementing the Live Well Domains to contribute, support and ensure alignment with the ICP’s five year plan and the ICB’s Joint Forward Plan Collective Ambitions for Suffolk and North East Essex

2. Supporting enablers across the Alliance including workforce, estates, digital, Primary Medical Care, Medicine Optimisation, Integrated Neighbourhood Teams and Connect

3. Using data to inform Alliance working focussing on the four Ipswich and East Suffolk Alliance priorities:

• Priority 1 – Reduction in management of hypertension
• Priority 2 – Reduction in falls & increased frailty management
• Priority 3 – Reduction in smoking
• Priority 4 – Reduction in Health Inequalities

CORE20PLUS5 highlights the need for ongoing prevention work and if driven by population health management data and approaches will be more effective than ever before. There are clear opportunities within Ipswich and East Suffolk to improve identified conditions, care pathways and health behaviours which would increase the likelihood of good health outcomes for many thousands of adults, children and young people. PLUS populations are those groups of people who may not be in the CORE 20 population, but who face inequalities in outcomes, access to, or the experience of care and whose needs must be explicitly recognised and met in order to reduce inequalities. The PLUS populations for Suffolk are:

• People from minority ethnic communities
• Coastal communities
• Rural communities
• People and groups facing the sharpest health inequalities in Suffolk

We will focus some of our efforts on our most deprived areas in Ipswich; Chantry, Gainsborough, The Triangle, Maple Park & Whitehouse. We will complete an Equality and Health Inequalities Impact Assessment (EHIIA) for the whole Alliance, Strategy and Governance and for each component programme of work.

Our Alliance is working with local partners to understand what more we can collectively do to change this. This includes voluntary and community partners working with hospital, GP practice, mental health and housing colleagues to enable people most in need, suffering or at risk of poor health and wellbeing, to have warm homes, warm meals and warm, joined up health and care services for young people, families and isolated elderly people, as close to home as possible
